For women who wish to apply to come to Destiny Grove, you will need to contact us by phone first (0249965558) and then download and complete this form. Form - Client Application and Assessment Form may 2008.pdf

It is a 15 page document that will help the team at Destiny Grove assess the needs of the client applicant. Once filled in, send it to us at PO Box 106, Clarence Town. NSW. 2321 and we will contact you within a week of recieving it to discuss availability.
